What’s Included?
- 2 regular cleanings with Fluoride

At the date of enrollment and upon renewal the fee is as follows:
$365 per adult (13 and older)
$275 per additional adult (13 and older)
$195 per dependent (3-12 years old)
Terms:
This is an annual non-refundable contract with full payment due at the time of service.Regular Cleanings are cleanings with no evidence of gum disease. This is non-negotiable. If a debridement is needed, a $80 charge will be required at the time of service. Patients with periodontal disease will require a Scaling and Root Planing Procedure and it will be discounted at $200/quad and will need periodontal maintenance at which 2 will be covered at no cost, and any additional will be charged at $130 per periodontal maintenance.
Fees are subject to change at the change of each contract year.
