At 58 Peaks Dental, we believe everyone deserves access to high-quality, comprehensive dental care—from preventative services and restorative treatments to cosmetic and advanced procedures. That’s why we offer multiple ways to make paying for your care simple and stress-free, including our in-house savings plan. Our in-house discount plan is an annual membership that gives patients discounted routine dentistry, including two cleanings with fluoride, a complete exam, emergency exams, all required X-rays, and 20% off all other dental work, with no deductibles, no maximums, and no waiting periods.

This is an annual non-refundable contract with full payment due at the time of service.
Regular Cleanings are cleanings with no evidence of gum disease. This is non-negotiable. If a debridement is needed, a $80 charge will be required at the time of service. Patients with periodontal disease will require a scaling and root planing procedure. It will be discounted at $200/quad. Periodontal patients will also need periodontal maintenance. Two periodontial visits will be covered at no cost, and any additional visits will be charged at $130 per visit. Fees are subject to change at the change of each contract year. After fully reading and understanding the terms, I agree to enlist herewith the contract.
